Use this questionnaire to help you have a place to start when connecting with your students’ parents for initial or upcoming IEP meetings. I find this helpful with getting a picture of what my students are doing in their home environments as well as what goal areas families would like to work on.

This questionnaire is most appropriate for students in early intervention, preshcool, transitional kindergarten, and possibly kindergarten. As always, you as the clinician can best decide what meets the needs of your clients/students based on their individual development.
